Teen Orphan Fiction/Fantasy Set in Winter

I read this book in middle school so that would around 2012-2014 (but I don't know the actual publishing date).
What I remember:
The book starts at an orphanage, in winter centering around 4 kids, 2 boys and 2 girls. One of this kids is ill and the other 3 get permission to leave so the.y con go see thier friend. At some point they decide to runaway I think and the orphanage send people to hunt them down. I think 2 of the kids are decended from nobility.
They kind split off into pairs and get separated. Over the course of weeks or months they run and hide. One boy ends up getting stuck in a gladiator type place where I think he gets the nickname mouse, and there's some crazy kid that hates him. I do t remember what happens to the 1st girl. The second pair of kids end up working for some old couple for a bit, but when they get caught up to by the hunters they run. At some point they break into a cabin, steal some clothes, sleep in the cabin a d leave just before the hunters get to them.
The kids start the converging arc where they all presupposes to Come together again, but something happens to the first boy. Like I said, he got tuck in an arena type place. Well, one day a massive fight breaks out between all the gladiators, and he decides he wants to take the chance to escape. As he is walking down the tunnel the crazy kid who hates him stops the boy and tries to fight. The boy tries to convince the crazy kid to let him go and just escape, but the crazy kid stabs him in the stomach with a sword and he dies curled up in the fetal postion. I think at least one of the other main characters find his dead body a d they're all in disbelief and sad.
That's where I stopped the book because I as mad that the boy died after fighting so much in the arena. I think the title had something g to do with "winter" or "north".
